Incorrect Parts Fitted
After Reviews of major defect reports, CASA advise that incorrect counter weight rollers were found fitted to a Lycoming IO-360.
In this case, the induced vibration caused a propeller blade tip to separate in flight.? This in turn led to an in-flight engine shut down and subsequent forced landing.? An eddy current check, indicated the development of a crack in the same area on the opposite blade.
CASA require all approved organisations to use the latest data available from the manufacturer.? All organisations must also ensure they have in place a quality system that will prevent this type of accident.
