Downloading and Submitting Forms
Download a form
To download a form you must have first completed your registration for CASA Self Service. If you have not yet registered go to How to sign up.
In the navigation, click on ‘Service Requests’ and then on ‘Download Forms’
The forms available for download are organised into categories. Select the category of the form that you are looking for; use the ‘arrow’ icons to expand and collapse categories.
A list of the forms available under the selected category will appear in the table below the categories list. Look through the table for the form that you would like to download, and then click the flag icon to the right to download that form.
If your browser asks you to ‘open’ or ‘save’ the form, select ‘save’ and keep a note of the folder you are saving the form to.
Open the form that you just downloaded from the folder it was saved to.
Fill out the form and follow any instructions.
Click the save button to save the completed form to your computer. Be sure to keep a record of where you save the file, as you will need to upload it from that folder to CASA Self Service.
Submitting a form
In the navigation, click on ‘Service Requests’ and then on ‘Submit Request’.
You can upload your form in one of two ways:
- By clicking ‘Select Form’ and browsing for the file, or
- By dragging and dropping the file inside the dashed border.
You will only be able to upload a form that has been downloaded from the CASA Self Service. If you need to upload supporting files, you can do so after you have first uploaded the form for your application.
Once you have uploaded a form for your application, you can upload additional supporting documents. You can upload additional documents in one of two ways:
- By clicking ‘Select File’ to browse for the file(s), or
- By dragging and dropping the file(s) inside the dashed border.
Click ‘Submit Request’ to submit your request for processing.
A submission confirmation screen will display which will provide you with a service request number (SR-######).
Viewing your service requests
You can view the progress of any requests that you have submitted via CASA Self Service.
In the navigation, click on ‘Service Requests’ and then on ‘My Requests’.
Note: At present you are only able to see requests that have been submitted online via CASA Self Service. Applications submitted by mail, email or fax will not be seen on the My Requests page.
