Service difficulty reports
There are two ways to submit your service difficulty report (SDR) or suspected unapproved parts report (SUP). You can fill in an online form or print the form off our website and post / fax / email it to us.
Using the online form
The SDR and SUP online form is our preferred method of receiving your reports. You can include attachments such as photos and diagrams with your report.
You have three hours to complete the online form. If it takes longer you’ll be logged out and might lose your information
Submitting a paper report
To submit a paper report, download the defect report form To submit a paper SDR, download the Adobe acrobat file, print, complete and return to CASA.
You can also download, print and complete the suspected unapproved part form and return it to us.
Defect report summaries
You can review and print our online defect report summaries by calendar year or monthly reports. You can also download a comma-separated file for your own analysis.
