The CAA (UK) has produced
CAP 712 - Safety management systems for commercial air transport operations (41 pages, 240Kb, April 2002) This is a useful, easy to read document.

CAP 730 (UK CAA) Safety Management Systems for Air Traffic Management.(38 pages, Adobe Acrobat, 194Kb, September 2002)

This Guide has been produced by the Air Traffic Services Standards Department and is intended to inform and aid the reader such that an effective Safety Management System can be delivered, appropriate to the size and scope of any particular operation, for managing safety. The guidance is almost identical to that appearing in the original guidance document (CAP 712) on which it is based.

Comprehensive information and guidance on the essential elements for creating a safe ground handling environment for aircraft and individuals.

Canada has looked at CASA's document and included a lot of the content in their version: Introduction to Safety Management Systems

The booklet Risk Management and Decision-Making in Civil Aviation describes Transport Canada's approach to risk management and decision-making. The booklet also covers the basic concepts of risk and risk management; highlights typical decision situations in civil aviation and outlines a decision-making process for managing risk.

If you want a more scientific and technical approach, then take your time to browse the large tomes put out by FAA and GAIN.

Keep in mind that FAA have been reviewing their Regulations and approach to system safety for 20 years. Their System Safety Handbook is extensive and can get technical. The link is to a web table of contents. The handbook consists of 17 chapters and 9 appendices availablle as Adobe Acrobat documents.

Global Aviation Information Network has produced Operator's Flight Safety Handbook Issue 2. This is also extensive.
