How to change your name on your CASA documents

Licence holders have a responsibility to tell CASA when they change their legal name.

Changing your legal name is a straightforward process.

You must submit to CASA a certified true copy of your name change document and a signed statutory declaration and CASA will update your personal details with your new name.

Types of name change documents

  • A re-issued birth certificate including the section that lists any previous names;
  • Certificate of Name Change;
  • Marriage certificate
  • Any other document, as agreed to by CASA (we recognise that some countries do not issue certificates of name change)

If you are notifying CASA of your name change at the same time as applying for a licence or other qualification, please attach the certified true copy of your name change document and statutory declaration to the application.

**If you are completing a CASA application form for a licence, ARN, ASIC or other application form, and have previously changed your name, please write your previous name/s on the application form and supply copies of your name change documentation.

(If there is insufficient space in the previous name field of any CASA form please provide this information on a covering letter.)

Please note that if you change your name you must have your medical certificate, ASIC, AVID, flight crew licence and/or aircraft registration documentation reissued. There are fees associated with the reissue of all these documents, except aircraft registration related documents.
