Online form (SDR or SUP)

Read the help page before completing your first report.

The online form is the preferred method of submitting reports. You can include attachments (photos, diagrams etc) with your report.

Please note: There is a time limit on your input. If it takes longer than 3 hours you will be logged out and you may lose your information

Defect Report form (PDF) To submit a paper SDR, download the Adobe acrobat file, print, complete and return to CASA.

Suspected Unapproved Part form (PDF) The form is at the back of CAAP 51-2. Download, print, complete and return to CASA.

Online Defect Report summaries You can view and print calendar year or monthly reports, or download a comma-separated file for your own analysis.
