AOC Holders Safety Questionnaire

Welcome to the AOC Holder Safety Questionnaire July 2009 for Charter and RPT operators. Please take the time to read the important information below before commencing the questionnaire.

Legal requirement

As a condition of your AOC you are obliged under the Civil Aviation Orders (CAOs 82.1, 82.3, 82.5, and 82.7) to ensure that the questionnaire is completed and submitted not later than 28 days after being asked by CASA. If you feel that you may not be able to reply to the questionnaire within that time period and you wish to apply for an extension to the deadline, you must do so before that date.

Completing the questionnaire online

You need a username and password to complete the questionnaire. You should have received these in an email on Monday 6 July 2009. These details were also sent by ordinary mail. If you have not received your username and password, please contact Beth Baldwin (contact details below).
