CASA widens investigation
The Civil Aviation Safety Authority has released its preliminary report of the investigation into the internal issues arising from the suspension, and subsequent conditional stay, of the Air Operator's Certificate of Aquatic Air Pty Ltd.
"I have decided that it is in the public interest to release the preliminary report of the investigation", the Director of Aviation Safety, Mick Toller said today.
"I want to stress that this is only a preliminary report which does not make any definitive findings, nor have the staff concerned had the opportunity, other than briefly, to comment or answer criticisms in the report.
"As a result, a more detailed investigation will be conducted to examine a number of specific issues, in accordance with the following terms of reference:
Conduct a full and complete review into the operations of Aquatic Air Pty Ltd trading as South Pacific Seaplanes, and the actions of CASA regarding such operations for the three years ending 31 July 1998.
"I have asked that the investigation team present its report to me through the Assistant Director, Aviation Safety Compliance, by 31 August at the latest."
Mr Toller said recommendations on any changes required to surveillance procedures or certificate action will form part of that report.
CASA continues to cooperate fully with the Bureau of Air Safety Investigation (BASI) in its investigation of the accident.
