Applying for an ASIC

Application form 639

Application form 639 should be used when applying for:

  • Student Pilot Licence
  • Aviation Security Identification Card (ASIC)
  • Aviation Security Status Check and Aviation Identification.

If you already hold an ASIC and are applying for a new ASIC, follow these instructions to ensure you have the correct form.

Before completing this form, make sure you have read Form 639i information sheet.

If you are also applying for a new flight crew licence, such as upgrading from a PPL to a CPL or getting a licence in another category aircraft, you need to submit the appropriate application form for that licence.
(Note this does not include the reprinting of an existing licence or applying for a photographic licence)

Complete the form and submit it by mail or in person to CASA’s Licensing and Registration Centre (CLARC) in Canberra, or in person to a CASA Field Office. Applications submitted to Field Offices will be forwarded to CLARC for processing.

If you are under 18 or hold a current ASIC and you are applying for a licence you do not need to complete the criminal records check section of the form.

Information on your form will be used for the aviation security status checks and to assess your eligibility for the licence.

You will need to prove your citizenship with your passport, birth certificate, or citizenship certificate. If you are not an Australian citizen you need to present your passport, visa details, list your arrival details and unless you are a permanent resident, state the date of your intended departure. Please note, if you hold a permanent residency visa you still need to show your citizenship and undergo the unlawful non-citizen check. All the requirements for proof of identity are on the application form.

You will have to attach two recent passport-sized photographs to the application form – the specifications are included on the application form. One of the photographs and the proof of identity documents need to be certified by the same person who certifies the proof of identification documents. The list of acceptable persons is on the information sheet.

If you have an adverse security assessment or you are found to be an unlawful non-citizen you will not be issued with a licence. If you already hold a licence your licence would either be suspended or cancelled.

The new regulations have appeal provisions.
