AOC Holders Safety Questionnaire

In December 2007 CASA sent a letter to most AOC holders in Australia about the AOC Holders Safety Questionnaire. Very large operators and foreign operators are not required to complete this questionnaire. The questionnaire will provide CASA with the safety data it requires to effectively perform its role.

Completing the questionnaire on-line

You need a username and password to complete the questionnaire. You should have received these in an email on Tuesday 5 February. Some operators, where we did not have an email address, have been sent the details by ordinary mail. If you have not received your username and password, please contact Natalie Johnson (contact details below).

Note: The password in the email has all letters in uppercase to distinguish the number zero "0" from the letter oh "O". The password in the form is not case-sensitive, you can enter letters in upper-case or lower-case.

You must submit the questionnaire by Wednesday the 5th of March, 2008.

What’s involved

The questionnaire asks you for information relating to your organisation's activities conducted during 2007. If your organisation did not operate in 2007, and/or does not intend to operate during 2008, you will only be asked to complete a very brief set of questions.

If you are the holder of more than one AOC, you should receive multiple usernames and you will need to complete a separate questionnaire for each AOC.

If your records are insufficient to give an exact answer to the any of the questionnaire's questions, please estimate the answers as accurately as possible.

You can save your answers and exit the questionnaire at any time. When you log back in again, the program will take you to where you got to last time.

Please remember that the PDF versions of the questionnaire contain many questions that may not apply in your particular case. The on-line questionnaire will only present the questions that are relevant to your operation.

Please remember that your answers should be received by CASA by Wednesday the 5th of March, 2008.

Legislative requirement

You are obliged under the Civil Aviation Orders (CAOs 82.1, 82.3, 82.5, and 82.7) to ensure that the questionnaire is completed and submitted not later than 28 days after being asked by CASA. If you feel that you may not be able to reply to the questionnaire within that time period and you wish to apply for an extension to the deadline, you must do so before that date.
