Online form for defect reports and suspected unapproved parts
The online form is the preferred method of submitting reports. You can include attachments (photos, diagrams etc) with your report.
Read the help page before completing your first
report.
Please note: There is a time limit on your input. If
it takes longer than 3 hours you will be logged out
and you may lose your information
Submit a paper report (SDR or SUP)
Defect Report form (PDF) To submit a paper SDR, download the Adobe acrobat file, print, complete and return to CASA.
Suspected Unapproved Part form (PDF) The form is at the back of CAAP 51-2. Download, print, complete and return to CASA.
